TRACK 1: Targeted Nutrition & Lifestyle Coaching
(Non-Medical)
This option is for families seeking expert guidance on specific dietary issues, performance nutrition, or general healthy eating habits, without the need for medical management of obesity or obesity medication prescriptions.
Nutrition Consultation Package
Ideal For: Picky eaters, athletes seeking performance nutrition, managing specific dietary needs (e.g., Celiac, IBD), or general healthy eating education.
Fee Structure: One-Time Package Fee: $499
Includes: One 75-Minute Initial Consultation with Dr. Muth (MD/RDN) and Two 30-Minute Follow-Up Sessions (used within 90 days of initial consult).
Access: Secure Messaging for clarification questions for 90 days post-initial consult.
TRACK 2: Comprehensive Obesity Medicine (Medical Management)
This option is for teens with obesity who require ongoing medical diagnosis, comorbidity management, and potential obesity prescription and monitoring.
Foundation
Ideal for: Teens/Families in the weight loss maintenance phase or those needing minimal medication titration.
Monthly membership fee: $149
Medical Visits with Dr. Muth Included: Bi-Monthly (Every 8 weeks)
Visit Duration: 30 Minutes

1. One-Time Enrollment Fee
A one-time, non-refundable enrollment fee of $199 per child applies to all new Comprehensive Obesity Medicine (Track 2) members. (This fee is waived for Track 1: Nutrition Consultations).

This fee covers the extensive initial medical and social history, chart review of outside records, and personalized treatment plan development required for the first 60-minute visit.
2. What Your Membership Includes (The Value)
Your fee covers the specialty physician time, access, and coaching services that insurance often restricts or denies:

Extended Telehealth Appointments: Sufficient time to discuss medical, nutritional, and behavioral goals.

Obesity Medication Management & Prescription (Track 2 Only): Careful monitoring and titration of medication dosage.

Virtual Check-ins: Secure, asynchronous communication with Dr. Muth and the care team.

Exclusive Resources: Access to our private digital library of recipes, meal guides, and educational videos, PLUS monthly live educational webinars with Dr. Muth and other pediatric nutrition and obesity experts.

No Copays: You will never pay a copay for a scheduled visit.
3. What Your Membership Does NOT Cover
The membership is not insurance and does not cover the costs billed by outside entities.
You must maintain separate major medical insurance for:

The Cost of Medications: Prescription drugs are billed to your major medical insurance or paid cash.

Laboratory Services: Blood work, imaging, and diagnostic tests are billed directly to your insurance by the lab (e.g., Quest, Labcorp).

Outside Specialist Referrals: Visits with cardiologists, endocrinologists, or surgeons.
4. Commitment & Cancellation
We believe in long-term partnership for chronic disease management.

Track 2 (Comprehensive): We request an initial three-month minimum commitment to allow the treatment plan and any medications to take effect. After the initial term, membership is month-to-month and can be canceled at any time with 30 days written notice.

Track 1 (Nutrition Consult): This is a one-time package fee and does not require a monthly commitment.
