Step 2. Choose your membership tier and complete your intake form.
 

Step 3. Your 60-minute initial visit with Dr. Muth. Comprehensive medical and nutrition history, lab review, medication evaluation, and a personalized care plan.
 

Step 4. Ongoing monthly or bi-monthly visits, direct messaging access, and lab monitoring — all calibrated to your teen's progress.

What you will still need:

Separate major medical insurance for labs (Quest/Labcorp), prescriptions, and specialist referrals

* HSA/FSA funds are accepted for patients with BMI ≥ 95th percentile

What Your Membership Includes

Your fee covers the specialty physician time, access, and nutrition services that insurance often restricts or denies:

  • No Copays: You will never pay a copay for a scheduled visit.

What Your Membership Does NOT Cover

The membership is not insurance and does not cover the costs billed by outside entities.

You must maintain separate major medical insurance for:

  • The Cost of Medications: Prescription drugs are billed to your major medical insurance or paid cash.

  • Laboratory Services: Blood work, imaging, and diagnostic tests are billed directly to your insurance by the lab (e.g., Quest, Labcorp).

  • Outside Specialist Referrals: Visits with cardiologists, endocrinologists, or surgeons.

Commitment & Cancellation

We believe in long-term partnership for chronic disease management.

  • Tier 2 and 3: We request an initial 3-month minimum commitment to allow the treatment plan and any medications to take effect. After the initial term, membership is month-to-month and can be canceled at any time with 30 days written notice.

  • Tier 1 (Nutrition Consult): This is a one-time package fee and does not require a monthly commitment.

  • Initial Consult: This is a one-time fee.
